TLC PROJECT DESCRIPTIONS

HOMESHARE

A living arrangement in a community household other than with natural family members. The home is owned or rented by the Homeshare provider. The consumer for whom the provider is reimbursed lives like a member of the household. Providers are screened, trained and supported by an intensive/outreach case management team consisting of a master level case manager, licensed registered nurse, psychiatrist and program secretary. Assisting Neighbor (skill development staff) and Community Companion (volunteers) may be added to the team as needed. Respite services are available to cover the providers' annual vacation, hourly or overnight needs. Respite providers must complete the same screening and training requirements as permanent providers.

All services of the local MHC are available, such as RILS, RPT, Supported Employment, transportation and so on. Medical and dental services and other community resources and services are provided based on the clients individual needs. Homeshare programs are available in the following mental health centers:

Crisis Stabilization Programs

Homeshare Enhanced Respite programs are short-term placement options for distressed clients to divert hospitalization while addressing the need for a safe and nurturing environment. Providers will receive extra training and support. Homeshare Enhanced Respite programs are available in the following mental Health centers:

SUPERVISED/SUPPORTED APARTMENTS:

A living arrangement in a community apartment complex with mental health center staff or with mental health staff available. A living arrangement in a community apartment setting with mental health center staff available on a daily basis. Consumers live alone or with a roommate in an apartment, and are supported by intensive/outreach case management services including but not limited to individual living skills training, social/recreational activities and crisis response.

The case management team comprises a master level case manager, licensed registered nurse, psychiatrist and program secretary. All services of the local MHC are available, such as RILS, RPT, Supported Employment, transportation and so on. Medical and dental services as well as other community resources and services are provided based on the clients individual needs. Supervised/Supported apartments are available in the following MHC:

Crisis Stabilization Programs

Highly supportive and structured apartment setting where clients in crisis receive intensive rehabilitative treatment and assistance from mental health staff and/or consumers. This is a short term placement option that intends to improve the client's level of functioning and return him/her to prior living situation. Clients will be assessed and determined to be in need of a highly supportive/ supervised environment. Intensive rehabilitative treatment will be offered focusing on individual living skills, medication monitoring, individual or family therapy, education, referral and linkage with community support systems. All other mental health services offered by the MHC are available, such as RILS, RPT, Supported Employment, transportation and so on. These programs are available in the following CMHC:

INTENSIVE RESIDENTIAL PROGRAMS (IRP):

A small group living arrangement licensed by DHEC and managed by the local mental health center. Center staff is on site 24 hours a day with a one to three staff to client ratio being the minimum.

The staff consists of a master level case manager, a registered nurse, mental health counselors, a psychiatrist ( as needed). Structured programming is provided seven days a week. This includes group therapy, living skills, leisure/recreational activities and individual counseling.

All services of the local MHC are available, such as RILS, RPT, Supported Employment, transportation and so on. Medical and dental services as well as other community resources and services are provided based on clients' individual needs. This type of residential program is available in the following MHC:

WACCAMAW CLOSELY SUPERVISED GROUP LIVING
(RCF LEVEL II):

A supported, serviced enhanced group living arrangement licensed by DHEC and under a MOA with the mental health center. Center staff is on site part of the day, and service delivery can be on-site or at the local MHC.

On site staff consist of a licensed registered nurse, licensed case manager, mental health counselors, a psychiatrist (on an as needed basis). Structured programming is provided seven days a week. This includes group therapy, living skills and individual counseling. Medical and dental services as well as other community resources and services are provided on the clients individual needs. This type of residential program is available in the following MHCS:

Crisis Stabilization Programs

A DHEC licensed group living arrangement focused on short term, hospital diversion for clients in need of supportive structured setting. Mental health staff are on site part of the day to provide assessment and treatment for clients in acute need. Services may include medication monitoring, individual and family therapy, education, living skills assessment, transportation, and referral/linkage with community support systems. DAY PROGRAMS

A therapeutic environment where clients can receive intensive monitoring, education, and rehabilitative activities of daily living during the day. All other services offered by the local MHC are available such as intensive case management. The following CMHC have this TLC Crisis program:

INTENSIVE CASE MANAGEMENT TEAMS

An assertive outreach treatment approach to clients in a crisis, or high recidivist who use the system for psychiatric or situational distress. Mental Health services may include: crisis management, medication administration and monitoring, treatment planning and formulation of stabilization needs, transportation, placement, and referral/linkage to community support systems. DETOXIFICATION PROGRAMS

A setting that is able to provide social and/or medical detoxification for clients who are dually diagnosed (Mentally Ill and Substance Abuse) and require a safe environment where clients can be monitored, assessed, and treated for precipitaling crisis. The following MHC have this program:
