Your Treatment Plan
A written plan for your treatment which
addresses your own needs, strengths, abilities and preferences will be prepared
by your therapist, counselor, and physician, with your participation. The plan
will be reviewed periodically for progress towards treatment goals and/or
discharge. Your treatment team will begin planning at your admission for your
improved level of functioning and for your discharge upon completion of your
goals.
Your First Visit
Your first visit will take approximately
two hours. If you have had treatment elsewhere, with your written consent, we
will contact former providers to send us a copy of your records. If you have
insurance, Medicaid, Medicare, etc., bring a copy of your insurance card so that
we can photocopy it for billing purposes. If you are participating in a managed
care organization, we will obtain preauthorization for your first visit. After
completing identification information, you will be asked for financial
information. If you are working, bring a copy of your most recent pay stub. You
will be asked to sign for consent to treatment. You will also be asked to sign
consent for the center to provide information to your insurance company for
reimbursement purposes only. You may also be given a symptom checklist to
complete. You will also be given a copy of our Client Orientation material and
asked to sign that you have received it. During your orientation, our staff will
familiarize you to the building, its exits, first aid kits, fire extinguishers,
& fire pull alarm stations. If there is ever an emergency while you are in the
building, please notify the nearest staff member. Your assessment will take
approximately one hour. If you need to be seen by a psychiatrist, we will
arrange for this appointment as soon as possible. Follow-up appointment
information will be provided at the end of the
assessment.
