TO:                 All Organizational Components

SUBJECT:     Medication at time of Discharge from a Facility

Consumers in our hospitals will receive medication when appropriate as part of their overall inpatient treatment regimen. The goal for the utilization of medication is to contribute to the reduction of psychiatric symptoms and to facilitate personal recovery. The continuance of medication, as appropriate, will be an integral part of all discharge planning activities.

If prescription medication is medically necessary and a limited supply is required to facilitate discharge, the consumer may receive up to a fourteen day supply of his/her medication. The consumer will receive sufficient instruction on the taking and safe storage of this medication.

DMH will periodically monitor prescribing patterns to identify opportunities to improve quality and continuity of patient care.

This Directive rescinds and supersedes South Carolina Department of Mental Health Directive No. 572-81, Prescription Drugs for Discharged Patients.
