The Bipolar Guidelines are part of the family of the VA Clinical Guidelines for Management of Persons with Psychosis and consist of explicit algorithms supplemented by annotations that explain the specific decision points and their basis in the scientific literature. The guidelines are organized into 5 modules: a Core Module for diagnosis and assignment to mood state plus 4 treatment modules (Manic/Hypomanic/Mixed Episode, Bipolar Depressive Episode, Rapid Cycling, and Bipolar Disorder With Psychotic Features). The modules specify particular diagnostic and treatment tasks at each step, including both somatotherapeutic and psychotherapeutic interventions. (J Clin Psychiatry 1999;60:9-21)

This site contains the "expert consensus" treatment guidelines (Tri-University Project) for bipolar disorder, schizophrenia, and obsessive-compulsive disorder. They are comprehensive, but emphasize medications.  

Medication Algorithms

This site, sponsored by the Texas Department of Mental Health and Mental Retardation, contains detailed treatment recommendations ("The Texas Medication Algorithm Project " or TMAP) for each of three major disorders (schizophrenia, or SCZ; bipolar disorder, or BPD; and major depressive disorder, or MDD).

Under its Evidence-Based Practice Program, the Agency for Health Care Policy and research (AHCPR) is developing scientific information for other agencies and organizations on which to base clinical guidelines, performance measures, and other quality improvement tools. This is Evidence Report/Technology Assessment Number 3 and it addresses pharmacotherapy for alcohol dependence.

Treatment of Schizophrenia

This important article, from Schizophrenia Bulletin, contains a very specific and comprehensive set of treatment guidelines for schizophrenia (referred to as "PORT") from the "Patient Outcomes Research Team" funded by NIMH.

Eighteen of the recommendations address antipsychotic agents and adjunctive pharmacotherapies; the remaining 12 address electroconvulsive therapy, psychological interventions, family interventions, vocational rehabilitation, and assertive community treatment/intensive case management.
